Set up a note-taking system for your loved one's doctor's appointments.

What You Should Know

A lot of info can come out of a doctor appointment. It may be hard to understand, even if the doctor writes it up. Therefore, it's important to create a solid system for taking and sharing notes on medical guidance. What's important is consistency and completeness. Here's a good start for what the notes should include: date, purpose of visit, follow-up questions for the doctor, symptoms, test results, prognosis, changes in medication, and things to watch for.

Steps To Take

  • Before appointments, prepare all questions.
  • Share notes with the team using email, Google docs, Evernote, etc.
  • Consider bringing on a volunteer "medical note taker" to help.
